| For your convenience we have posted the
Assessment Report Title Page & Summary Form and the Guidelines for
Reporting Exploration and Development Work Checklist for viewing or printing. The Checklist documents common problems
encountered when reviewing assessment reports for approval. Both these documents are
in .PDF format and require the free Adobe Acrobat Reader to access them. You
can print the Assessment Report Title Page & Summary Form for inclusion in the front
of your assessment report. Note that the Checklist is only a summary -
refer to the Order in Council 1161-04 for complete details.
You can now key your Assessment Report Title
Page & Summary Form information onto the form on the screen and print it for inclusion in your report. Simply tab to
each field or use the tab key to go down to the next line. NOTE: If your keyed information goes off the page
it will not print out. Once you have keyed in your information click on the Print Form button at the bottom of the
page. This .PDF form cannot be saved.
